Access to best-in-class sub-specialty care at Northwestern Lake Forest Hospital (patients have the option to go to the hospital of their choice if they have relationships there.)

Early morning appointments or late evening appointments are available.

​

Included in the membership are minor procedures like nebulizer treatments, EKGs, urinalysis, flu / COVID-19 / strep tests.

Once you pay the membership fee, there are no hidden costs. Visits are unlimited and you can cancel your membership at any time. (We do require a 3-month minimum for the membership).

We have three membership prices.​

  • Individuals: $215/month

  • Couples: $395/month

  • College Students: $100/month

​

The following services are included in the monthly fee:

  • Complete a comprehensive review of your medical history.

  • Collaborative plan of care.

  • In-person office visits and virtual visits (when feasible and advisable) via the secure, HIPAA-compliant platform that we
    use in our Practice.

  • Management of chronic conditions.

  • Timely visits to address illness.

  • An annual comprehensive physical.

  • Coordination of medical care with your specialists.

  • Coordination of medical care with your hospital team.

  • Access to our extensive referral network.

  • Tests completed in our office, also known as Point of Care (POC) testing, with
    immediately available results (such as rapid strep, rapid flu, rapid covid, glucose, urinalysis).

  • ECGs

  • Depression Screening

  • Cognitive Screening

  • Vision Screening

  • The convenience of in-house lab draws and vaccines (billed directly to your insurance by the Northwestern Medicine lab or Vaxcare, respectively).
